We are not contracted with insurance companies. This is an out-of-pocket practice, with payment expected at the time of service. We may offer payment plans for some services.
Representations

Governing Law
You agree that: (i) the Site shall be deemed solely based in Oregon State; and (ii) the Site shall be deemed passive, meaning it does not give rise to personal jurisdiction over Gut Revival Nutrition, either specific or general, in jurisdictions other than Oregon State. If you have any dispute with the Gut Revival Nutrition or Maya Rose that is not barred by the Terms and Conditions, the dispute will be governed by the laws of the State of Oregon without regard to its conflict of law provisions. You hereby submit to personal jurisdiction by and venue in the state and federal courts of Oregon. Regardless of any statute or law to the contrary, you agree that any claim or cause of action arising out of or related to use of the Site or the Terms and Conditions must be filed within one (1) year after such claim or cause of action arose, or it will be forever barred.
Class Action Waiver
You may only resolve disputes with us on an individual basis, and may not bring a claim as a plaintiff or a class member in a class, consolidated, or representative action. Class arbitrations, class actions, private attorney general actions, and consolidation with other arbitrations aren’t allowed.
The arbitrator may not consolidate more than one person’s claims, and may not otherwise preside over any form of a class or representative proceeding or claims (such as a class action, consolidated action or private attorney general action) unless all relevant parties specifically agree to do so following initiation of the arbitration.
Termination
We may cancel or terminate your right to use the Site or any part of the Site at any time without notice. In the event of cancellation/termination, you are no longer permitted to access the part of the Site affected by such cancellation/termination.
